1   Balance Test
Full tandem stance during the SPPB balance test.

Setup

  • Stand near wall. Shoes on.
  • Demonstrate each stance before timing.
  • Arms hang freely; do not hold participant.

Procedure

  • Side-by-side: Medial borders touching. Hold 10 s. If <10 s → score 0, stop.
  • Semi-tandem: Side of foot against big toe. Hold 10 s. If <10 s → score 1, stop.
  • Full tandem: Heel against toes. Time up to 10 s.

2   Gait Speed Test
Marked 4-meter walking course for the SPPB gait speed test.

Setup

  • Mark 4-meter course on flat surface.
  • Participant may use usual walking aid.
  • Start from static position behind start line.

Procedure

  • "Walk at your usual pace."
  • Start timing when participant begins walking.
  • Stop when leading foot crosses end line.
  • Two trials; use faster time.

3   Chair Stand Test
Correct starting position with arms folded for the SPPB chair stand test.

Setup

  • Firm chair ~43–45 cm seat height, no armrests.
  • Arms folded across chest; feet flat on floor.
  • Trial rise (no arms) to verify ability.

Procedure

  • "Rise fully and sit down 5 times, as quickly as you safely can."
  • Start timing on first rise.
  • Stop on full stand of 5th rise.
  • Score 0 if arms used or >60 s.
